Abstract: Intradiploic encephalocele is an uncommon condition, with around 20 cases reported in scientific literature. It involves the protrusion of brain tissue through the lytic destruction of the inner plate of the skull. Both MRI and CT scans play a crucial role in diagnosis and differentiation from other skull lytic disorders. Here, we present a rare case of intradiploic encephalocele located in the occipital bone containing cerebellar folia. The patient initially experienced only mild headaches. Differential diagnosis should encompass various calvarial lytic lesions, some of which may appear benign, however, certain lytic conditions can present serious or life-threatening risks and should not be overlooked. In this report, we underscore the importance of differential diagnosis, with a primary emphasis on neuroimaging characteristics, to facilitate accurate diagnostics and appropriate management of this rare condition.
